Corrective measures also were not taken when the unit's steam-bypass control system failed in August 1988, according to the report.

Regarding the third system cited in the report, preventive maintenance and testing on the reactor's emergency-lighting system was "improperly waived" for 15 consecutive months, the report said. A emergency-lighting failure forced employees trying to fix the valve problems in total darkness. Failure to remedy these items was described as "particularly disturbing."
